Terms & Conditions*
- 1. I am adopting the pet described below through Gabby's Animal Rescue, Inc. The pet is being adopted as a companion and will live as a family member with me inside my home.
- 2. I agree that a representative of Gabby's Animal Rescue, Inc. may make periodic inspections of the pet, my residence and the pet’s living conditions on reasonable notice. I further agree to surrender the pet to Gabby's Animal Rescue, Inc.’s representative if for any reason he/she considers the conditions provided by me for the pet to be unsatisfactory.
- 3. I agree to immediately notify Gabby's Animal Rescue, Inc. if the pet is lost or stolen. I further agree, in the event the animal is lost, that I will make every reasonable effort to attempt to locate and claim the animal, and to immediately retrieve the pet from any public pound or animal shelter when notified the animal is being held at any such location and to pay any costs associated with its recovery.
- 4. I understand that Gabby’s Animal Rescue, Inc. has had the companion spayed/neutered, administrated vaccines that were due before the time of adoption, de-wormed, flea treatment applied, and in most cases micro-chipped. The microchip DOES NOT include any fees incurred when new owner registers the chip.
- 5. I agree to take the companion to a veterinarian within 14 days for a wellness examination AT MY OWN EXPENSE. Should the companion be diagnosed with a medical condition requiring treatment, you may return the animal to Gabby’s Animal Rescue to receive a full refund. If you choose not to have the animal examined within 14 days and the animal is to be diagnosed with a medical condition later on you will NOT receive a refund.
- 6. I understand that Gabby’s Animal Rescue cannot guarantee the health, temperament or training of the companion described below and hereby agree to release Gabby’s Animal Rescue from all liability once the companion is in my possession.
- 7. I agree to provide proper food, water, adequate shelter, and kind treatment at all times for my companion. I agree to take my adopted companion to the veterinarian to receive annual checkups and to remain current on vaccines.
- 8. If adopting a canine companion, I agree that it will NOT have the ears or tail docked/altered.
- 9. I agree to license the companion in compliance with the laws and ordinances in force in the municipality in which I reside.
- 10. I agree to return the companion to Gabby’s Animal Rescue if I decide at any time that I can no longer keep the companion. I agree not to allow the companion to be used for medical or other experimental purposes.
- 11. I hereby acknowledge that the information provided above is accurate and truthful. I am willing to provide identification and/or documents to support this information. I agree to have Gabby’s Animal Rescue visit my home and/or contact my landlord if need be.
- 12. I understand that the adoption fee is NOT refundable for any reason except for the situation stated on line 5.
